To Participate in Athletics During the 2026-27 School Year, You Must Do the Following:

  1. Log into the Genesis Parent Module to sign up for the sport of interest, then read and sign all agreements/consents.
  2. Download or obtain a Sport Physical Packet from the Athletics Forms website, which contains the Athletic Consent form and the Physical Exam forms. A parent or guardian must complete the Consent Form and the health history portion of the physical forms. These are the only forms that can be used.
  3. Get a physical exam done by your Primary Care Provider (MD, DO, APN, or PA). Please bring the completed health history form to the exam. The provider must complete and document all parts of the exam. Physicals are valid for 365 days. All athletes must have a current valid physical to try out.
  4. Free sports physicals will be available at Piscataway High School on June 2 (girls) and June 3 (boys) for students of families who are experiencing financial hardship, do not have a private healthcare provider, or whose medical insurance will not cover a physical at this time. To participate in the free sports physicals, students must bring a fully complete sports physical packet with all necessary pages completed and signed by a parent/guardian.

  5. Submit the Sports Physical Packet to the school nurse prior to the due date. Please note once tryouts have started, no more students will be accepted to that team.

Fall 2026 Try-out Information

MS Football - Try-outs start on Tuesday, August 11th at PHS - Green Acres - 7:30am-9:30am

MS Girls Volleyball - Try-outs are Monday August 17th- Thursday, August 20th at PHS - Anthony Gym - 11am-1pm

All other MS fall sports Girls & Boys Soccer, Cheerleading, and Co-ed Cross Country will start try-outs on Tuesday, September 8th.  

MS tryouts will be held at the respective sport's school after school until 4:30pm.  Students will be provided transportation from their MS to the sports' respective school after school.  Students must be picked up at the school where the student is trying out at 4:30pm.

All students must be on the medically cleared list and have completed the online Impact concussion test to tryout.

  1. The athlete MUST have a CURRENT* sports physical turned into the school nurse by the due dates listed below. Sports physicals MUST be cleared before the first day of tryouts.
  • *Sports physicals are valid for 365 days from the first day of tryouts.
  • If a student played a previous sport and the sports physical is still valid, the Health History Update Packet from the Athletic Forms website must be completed and turned it into the school nurse by the Due Dates listed below. This includes an athletics consent form and a Health History Update Questionnaire.
  • The School Doctor will review ALL forms submitted for Athletics. It may take up to 2 weeks for athletes to be cleared after forms are submitted to the school. Any physicals handed in after the due dates may not be cleared in time for tryouts

Sports Physical Packet Due Dates:

  • Fall Sports – June 3
  • Winter Sports - October 5
  • Spring Sports - February 1

  • Sports Physical Forms are also available in the Nurse’s Office, PHS Athletic Training Room, and PHS Athletics Office.
  • Any questions regarding sports physicals and health history update forms should be directed to the school nurse.
  • During July and August, sports physical packets can be dropped off at PHS - Anthony main office - Monday-Thursday, 8 a.m.- 2 p.m.

The athlete MUST take the ImPact concussion baseline test.

  • Baseline concussion testing is required for all Piscataway student-athletes interesting in playing a sport.
  • All Grade 6, 8, 9, and 11 students, or anyone new to athletics, are required to test.
  • Students can take the ImPACT test at home.

  • Testing must be completed prior to your child's sport season.

NJSIAA Opiate Form and Video Policy -  It is a requirement for all athletes to view the NJSIAA Opiate Policy video.
