Asthma Action Plan
If your child has asthma and uses an inhaler please download the form to be completed by your physician.
** Please note** If your child has ever had a history of asthma but no longer uses an inhaler, your physician must note that on a script which must be handed in along with the  health history form.

**Please note** If you think that your physical is still valid for the next athletic season please contact the grade 9/10 nurse (ext 2288) or Grade 11/12 nurse (ext 2240)

NJSIAA Opiate Form and Video Policy
It is a requirement for all sports to view the NJSIAA Opiate Policy video. Please view video and return completion paperwork to you school nurse immediately.

School MD will review ALL forms submitted for Athletics
