All sports/health forms are located at the bottom of this page.

All forms in the Sports Physical Packet must be submitted before due dates below for the 2026-2027 school year. The school doctor will review ALL forms submitted for Athletics. It may take up to 2 weeks for athletes to be cleared after forms are submitted to the school. Any physicals handed in after the Due Dates may not be cleared in time for tryouts.

Sports Physical Packet Due Dates:
•    Fall Sports – Friday, June 3 
•    Winter Sports - Wednesday, October 5
•    Spring Sports - Monday, February 1

Please note: If you think that your athlete’s physical is still valid for the next athletic season, please contact the Grade 9/10 nurse (ext. 2288) or Grade 11/12 nurse (ext. 2240)

Asthma: If your child has asthma and uses an inhaler please download the asthma action plan form from the list below to be completed by your physician. If your child has ever had a history of asthma but no longer uses an inhaler, your physician must note that on a prescription, which must be handed in along with the health history form.
